The Michigan Ministries of Ascension Health include Borgess Health in Kalamazoo, Genesys Health System in Grand Blanc, St. John Providence Health System in Metro Detroit, St. Joseph Health System in Tawas and St. Mary's of Michigan in Saginaw. Reverence's statewide headquarters is centrally-based at Genesys in Grand Blanc, however, clinical services and staff remain at each local Health Ministry.

Job
Summary: Performs liaison activities. Assesses, plans coordinates referrals,
utilizing knowledge of criteria for reimbursement and qualifying
criteria for facility admissions. Facilitates smooth transition into or
out of appropriate facility. Coordinates referrals. Understands
services/programs and regulatory/accreditation requirements to support
transition of care. Participates in obtaining appropriate insurance
authorization for service. Builds trust with referral sources by
providing personalized customer service. Promotes collaboration and
coordination between the hospital and home health/hospice agency to
achieve continuity of care and utilize health resources effectively. .
MUST BE WILLING TO TRAVEL
Required:
3 -5 years years experience in acute care nursing required
Current RN License in the State of Michigan
Current Michigan Driver''s License
Current Auto Insurance
Associates degree in Nursing
Good written and oral communication skills
Ability to work with diverse populations
Good organizational skills
Demonstrates flexibility and adaptability to change
Familiar with computer
Preferred:
Bachelors degree in Nursing
Oncology/Hospice experienceNursing
Organization: Reverence Home Health & HospiceEstimated Salary: $20 to $28 per hour based on qualifications.
